This form is the first step in the process of beginning Online Giving at your parish. Please fill in all of the fields completely to expedite the approval process. Once we receive your form, we will populate the application and email it to you for verification and signatures.  You will need to fax back a copy of a voided check with the final application as well.

The explanations below should be read prior to completing the form. All fields on the form are required.

  • Legal Business Name: This must be identical to the Church name on your Federal Tax ID. If the name and the federal ID do not match, the approval process can be delayed significantly.
  • Primary Contact: This person will be the primary contact for Online Giving and the reports and questions will be directed to this person. Automated e-mails from the system will also go to this person. The primary contact does not have to be listed as an officer.
  • Church Founded Date: If this date is not available, please estimate as closely as possible (MM/YY).
  • Officer/Financial Administrator: This would be the person who has responsibility for the banking and financial records and can sign for the church. One person is required and the second one is optional.
  • Depository Account: This is the account where deposits will be made.
  • Fee Account: This is the account from which the setup & monthly fees will be drawn.

Text to Give Text to Give is an add-on feature to the standard Online Giving Program that allows donors to interact with the Online Giving System via Text Message to set-up special one-time gifts.

USB Swipe Card Reader This device connects to a desktop or laptop computer and will only 'swipe' credit cards. All transactions process at the same rate as Online Giving. There is a one-time fee of $65 to purchase the reader and no additional monthly fees.

EMV - Swipe Card Readers Swipe card readers can be used to take payments in the office, bookstore or on a mobile device during fundraising. These readers are Bluetooth enabled and able to be used with any device that can connect via Bluetooth. They read chip cards as well as swipe cards. EMV Card readers incur an additional $9.95 monthly fee plus an additional $0.02 per transaction, percentage fees remain the same.
